These changes fix a couple of minor issues recently introduced by myself ^_^

  • use mikeal/watch to watch directory trees
  • prevent from watching the same path multiple times
  • cleanup dynamically generated file after it is no longer needed
  • remove dependency no longer used: combine-streams
